We are a group of clinicians and support staff that believe physical therapy can be done better. The current orthopedic model is broken and starting in 2006, we have set out to change it. We believe in delivering the most comprehensive PT specialist care to you, in a home town caring manner. You will not find a collection of more dedicated professionals with as much understanding of postural control, gait, systemic respiration, and stability dysfunction in the country. The professionals listed below are ready to help you understand why you are having pain and to teach you how to restore your ability to live.
Our Story
Dr. Sean Fitzgerald MPT DPT PRC EPPM
Sean holds bachelors degrees in Health Science and Biology, a Masters degree of Physical Therapy from the University of Vermont, and a Doctorate of Physical Therapy from EIM Institue for Health Professions. Sean is currently 1 of only 150 Postural Restoration Certified therapists in the country. Sean is a member of the American Physical Therapy Association in the Sports PT and Private Practice sections as well as The Performing Arts Special Interest Group (PASIG) and the EIM PT Executives Masterminds group. Sean is also a proud member of the Performing Arts Medical Association (PAMA) and provides services for the St. Michael's Playhouse and numerous local performing artist.In addition to athletes and musicians, Sean enjoys working with patients of all ages and abilities.

​Sean grew up in Jericho VT and is happy to be able to return to that community, providing clinical service in his childhood home town as well as in Essex Junction where he resides with his wife and 2 children.

Dr Katie Swanson DPT
Katie is a native Vermonter and graduate of UVM earning her doctorate in Physical Therapy. Katie is excited to join our team again as she was on our staff for her final 10-week clinical rotation this past winter in our Jericho clinic. Katie has already completed over 40 hours of post graduate clinical education as she becomes a postural control and stability dysfunction specialist with emphasis on women’s health. Katie looks forward to continuing to providing care to her patients in the Jericho clinic.

Dr John Earls DPT
Johnny is also a native Vermonter and graduated from UVM with his doctorate in Physical Therapy. Johnny is a neuro-mechanical specialist and completed his last 10-week rotation at our Essex clinic. Johnny has completed over 40 hours of post graduate clinical education as he pursues advanced certification. Johnny looks forward to continuing care of his patients in both the Jericho and Essex clinics with particular interest in patho-mechanics, stability dysfunction, injury prevention, and respiratory influences on postural control.

Kristin Fitzgerald Practice Manager
Kristin graduated from Saint Michael's College with a bachelors degree in Journalism/Public Relations. Utilizing her previous professional experience of 20 years as the Senior Associate Director for Admission at Saint Michael's, she coordinates the marketing activities and community outreach for Transitions Physical Therapy. She also oversees the daily operations of the office including
scheduling, billing, insurance liaison and supervising the staff.

Kristin lives in Essex Junction with her husband and 2 children. She enjoys reading, gardening and traveling with her family.

Elise Minadeo MSPT
Elise holds a Bachelors degree in biology from the University of Vermont and a Masters of Science in Physical Therapy from Columbia University. Elise is a member of the American Physical Therapy Association as well as the Orthopedic and Sports Physical Therapy sections.
Elise’s background is rooted in geriatric physical therapy, but more recently she has pursued outpatient orthopedics through continuing education courses with special interests in Postural Restoration, kinematics and biomechanics of running, and treatment and prevention of running injuries.
Elise lives in Essex with her husband and 3 children. She enjoys
running, traveling and spending time with her active family.

Kristin Liberman, Care Coordinator - Jericho Clinic
Kristin is originally from Maine and attended Southern Maine Technical College as well as Oregon Health and Science University. She worked in the dental industry for 15 years in customer service prior to leaving the work force to be a stay-at-home mom to her three children.
Kristin lives in Jericho with her husband, children and a menagerie of pets. She enjoys many forms of exercise and has been a dance instructor for 15 years. Please feel free to call Kristin at the Jericho office with your questions.

Allie Gratton, Care Coordinator - Essex Clinic
Allie is a native of Essex Junction who graduated from Saint Michael's College with a Bachelor's degree in Business Administration. Prior to joining Transitions Physical Therapy, she was a Member Services Representative at Planet Fitness. Allie is an avid runner who is always looking for that next big race and enjoys sharing her running expertise as an assistant x-country coach at Essex High School. Allie also enjoys reading and has been known to share her excellent baked goods with the staff! Please don't hesitate to call Allie at the Essex office to reach her with any questions.
